Chebucto Community Health Team For
Communities from Armdale to the Pennants
The Chebucto Community Health Team will support individuals and families to build knowledge, confidence, and skills to help make healthy lifestyle choices and better prevent and manage ongoing health conditions.
Located in the South Centre Mall, the Community Health Team staff will be offering a variety of wellness programming, such as personal wellness assessments and group sessions on healthy eating, being active, setting health goals, emotional wellness and managing stress, body weight, blood pressure, and cholesterol levels. Additional programs will be offered over time.
The Community Health Team will also work collaboratively with family physicians, other health providers, and community-based services to help link residents with other programs and supports that will help support individuals to be as healthy as possible. All the programs and services offered by the Community Health Team are free.
