Civic Address of Alarm:
  • the actual address (civic number) of the alarm - the street address where the alarm is located (incl. city/town/community and postal code). *not the mailing address.

Alarm Type: (you may select only one option)
  • Intrusion:
    If your alarm notifies the police, monitoring company, or make loud noises when someone is trying to gain uninvited-entry to the premises.

  • Holdup:
    If your alarm notifies the police when a hold up is in progress (i.e. a panic button). Possibly a "silent alarm".

  • Medical:
    If your alarm notifies an appropriate emergency contact when you have a medical emergency.

Fire Alarm Activated: (yes or no option)
  • Does your alarm activate when there is a fire?

Owner/Tenant/Business Owner Information:
  • Assessment #:
    Property assessment number - obtainable from property owner.

  • Last Name:
    Owner/Tenant/Business Owner's last name.

  • First Name:
    Owner/Tenant/Business Owner's first name.

  • Number:
    Street number of premis.

  • Unit:
    Apartment/Unit number (if applicable).

  • Street:
    Street name on which the premis is located.

  • City/Town:
    City/Town in which the premis is located.

  • Province:
    Province in which the premis is located.

  • Postal Code:
    Postal code of the premis.

List of Contact Persons:
  • Name and phone number of additional contact should the main contact be unavailable.

  • Should the main contact be a tenant, the landloard's name must be listed as the first additional contact.

Alarm Company: (Company monitoring the alarm)
  • The name and phone number of the company that monitors the alarm at this location. * If your alarm is not monitored, state this in place of a name.
